Please note that as of March 14, 2008 it is
mandatory that every new client complete and return,
both the
Toxicity Questionnaire
prior to shipping your products
for the 21 Day Body Purification Program.
This ensures that, not only do you qualify to take
part in the program, but also which, if any,
symptoms are present and to what degree they are
playing a role in your current state of health.
Please be sure to include any medications you may be
taking such as (Coumadin).
Once we receive a copy of your forms and evaluate
